Evaluation of the Single Antigen Histo Spot HLA Antibody System as a comparison tool against existing single antigen bead assays (#101)

The Histo Spot HLA antibody system (BAG Diagnostics GmbH) test systems is a multiplex immunodiagnostic microarray which is performed on the automated Mr Spot Processor 2.0. Our aim was to compare this assay against existing single antigen bead technologies to determine suitability for adoption into a clinical laboratory setting to assist with antibody assignments for solid organ transplant patients. A cohort of 50 patients were identified for each of Class I and Class II respectively. Patient samples were chosen because the antibody profiles were difficult to assign using Lifecodes LSA (Immucor, USA) and LABScreen SAG (ThermoFisher, USA) assays. The samples consisted of highly sensitized patients, random reactivity with no clear antibody profiles, high background or disparate reactivity between the two Luminex kits.  Processing of selected samples was performed as per manufacturer’s instructions and antibody profiles determined using HistoMatch analysis software (BAG Diagnostics). Initial testing shows that a third platform may be useful but regardless of platform/kit, the antibody profiles for some serum samples are not clear-cut or are difficult to resolve. The Histo Spot HLA antibody system, when automated on the Mr Spot Processor, is a user-friendly adjunct to the current SAG bead assays available and would be suitable as an additional tool when assigning unacceptable antigens for organ transplantation.
